Residency Admission
- Completed MD / dental / pharmacy degree
- State licensing examination result (KROK 3)
- Specialty preference form
- Two recommendation letters
PhD / Doctoral Admission
- Master's degree or equivalent
- Research proposal (5,000–10,000 words)
- CV and publications list
- Supervisor agreement letter
